Service Awards
|
The Service Award Program recognizes
employees' Total State service in increments of five years through retirement.
For each five-year increment, the program offers a collection of items from
which eligible recipients may select.
For your convenience, you may view the service awards selections
by clicking on the the links below:
Please note the information below in your selection of a service award.

If you have completed 5, 10, 15, 20, 25, 30, 35
or 40 years of service and your anniversary date is before September
15, 2009, you are eligible to receive a service award.

Included in your packet of information, you will find a colorful brochure, with a selection of service awards to choose from. You may choose one of the service awards shown. Please make your selection and return the information to Barbara Greene, Human Resources Department by Friday July 31, 2009.

You are encouraged to select one award from your current service award level. However, if you are unable to select a service award from your current year selection, you may select a service award from any earlier award year. EXAMPLE: a 20 year employee may select one award from the 20 year selection
OR one award from either 5, 10, or 15 year level.

Please do not submit your service awards selection directly to CA Short Company. All orders
must be submitted through your Employee Recognition Program Coordinator.

It is important to return your form with your service award selection to the Human Resources Department by the July 31, 2009 deadline.
