This application form serves as authorization to make the change to University records and therefore must be signed by the employee. Changes will be entered into the automated personnel/payroll system. It is the responsibility of the employee to contact the appropriate providers directly. This will serve as an immediate verification of the correction in an expedient manner. The application indicates names and telephone numbers for reference. |
